Life Long Care

This community based program looks at providing care and services to urban Indigenous community members of all ages–from birth to elder. Specialized supports for those with physical disabilities, health issues, developmental issues, or who are frail and/or elderly.

The goal is to assist participants in stabilizing their health. Incorporating these aspects of life:

  • Physical
  • Mental
  • Emotional
  • and Spiritual

The Life Long Care Program is able to provide assistance in the following areas:

  • Medical escorts
  • Referrals to community services & programs
  • Preparing forms and applications
  • Home visits / friendly visits
  • Security check in / in person or over the phone
  • Caregiver Support services (respite & training)
  • Advocacy with community agencies
  • Access to cultural activities, traditional foods, ceremonies, Elders and Healers
  • Assistance with housing issues
  • Transitioning from ODSP to Old Age Pension
  • Assist in obtaining medical equipment & supplies
